By using the HolistiK12 platform, you agree to conduct transactions electronically, including but not limited to signing permission slips, consent forms, and other school-related documents.

Electronic signatures have the same legal effect as handwritten signatures. When you type your name in a signature field on this platform, you are creating a legally binding electronic signature under the federal Electronic Signatures in Global and National Commerce Act (E-SIGN Act) and the Texas Uniform Electronic Transactions Act (Texas Business and Commerce Code Chapter 322).

Your electronic signature indicates that you have read, understand, and agree to the terms of the document you are signing. All electronic signatures are logged with timestamps and associated with your authenticated user account.

You have the right to refuse to sign documents electronically. Under Texas UETA Section 322.005(c), this right cannot be waived.

If you prefer to complete any form on paper rather than electronically, you may request a paper copy from your school office at any time. Paper forms will be processed with the same validity as electronic signatures.

To request a paper form, contact your child's school office directly. School staff can provide paper copies of any permission slip or consent form available on this platform.

Account Security: You are responsible for maintaining the confidentiality of your account credentials. Notify your school immediately if you believe your account has been compromised.

Accurate Information: You agree to provide accurate and complete information when using this platform, including when signing permission slips and consent forms.

Authorized Use: You may only sign documents on behalf of children for whom you have legal authority (as a parent or legal guardian).

Record Retention: Electronic records of your signatures and transactions are retained in accordance with applicable state and federal requirements, including FERPA regulations for student records.
